After submitting a contact form, we will contact you to discuss our nurse coaching services. After this communication, your information submitted through the contact form is retained however this information is kept confidential and is never disclosed or provided to a third party. Your right to privacy is respected at all times.
As a natural part of the health coaching process, we collect basic demographic information such as name, age, and city of residence. By necessity, we also obtain a health history, information on your lifestyle habits, and medical records from pertinent physicians/providers in order to guide and coach our clients appropriately. This information, along with a thorough, individualized coaching plan, is kept within a secure client record. Except for instances where disclosure is required by law, your information is never shared with anyone… period.
Your personal and health information is kept confidential as we respect and support your right to privacy. This is a moral and ethical obligation that we are happy to follow. In addition, certain privacy laws govern the use and protection of health information. Our internal privacy practices align with and follow all privacy laws.
Any disclosure of your personal or health information outside of what is legally required is only done so with your expressed consent. As our client, you control how your information is used. If you authorize the disclosure of your information for any reason, you retain the right to revoke said authorization at any time and for any reason.
With few exceptions, you retain the right to review any and all of your health information that is kept as a part of our health coaching process. Exceptions to the right to review health information include instances where said disclosure is restricted by law or could result in harm or injury to the client or a third party. If you would like to review your health information, please provide us with a written request along with a copy of your identification card to ensure that the request is being processed and distributed to the appropriate individual as required by patient privacy laws and internal policies. Your information will be provided in either electronic or hard copy form and sent via postal mail to ensure privacy and confidentiality. Information can be sent via email by only with your expressed, written permission to do so.
All health-related organizations incur the risk of a data breach. In the event of a data breach, you will be notified in accordance with the laws and regulations that govern such event.
If you feel the need exists for an amendment to your personal or health information, you retain the right to request an amendment. Any request must be provided in writing and accompanied by a copy of identification. Such request will be considered by leadership and in the event the request is denied, you retain the right to file a statement of disagreement that will be added to and become a part of your client file.
If you feel the need, you may file a complaint with the Secretary of Health and Human Services if you feel that your right to privacy has been violated. We encourage you to contact us first so that we can discuss the matter and seek resolution as we are a client-centric organization that cares about those we work with.
